چکیده
Data on the feasibility of left bundle branch area pacing (LBBAP) and atrioventricular node (AVN) ablation in patients with pulmonary hypertension (PH) are limited. This study investigated characteristics outcomes LBBAP AVN PH. is a retrospective PH AF who underwent at Cleveland Clinic between January 1, 2019 August 2022. Pre-procedural, post-procedural, follow-up patient data were obtained. A total 11 (mean age 74±10 years old, 100% female) concomitant included. The etiology was Group III all (55% had COPD 91% restrictive lung disease), average arterial systolic pressure (PASP) 42 mmHg by echocardiogram, FEV1 61.9±17.4% predicted, FVC 59.9±7.8% predicted. Baseline ejection fraction 57±10%, QRS duration 96±20 ms. Concomitant successful patients. Final implant included: 124±8 ms, ventricular activation time 73.7±5.4 ms; threshold 0.6±0.2 mV @ 0.4 ms 0.7±0.2 follow-up; impedance 618±119 ohms 545±145 follow-up. Prior to procedure, number or heart failure (HF) hospitalizations 24 months prior procedure 4.9±5.3, 64% antiarrhythmic drugs, either beta-blockers calcium channel blockers. During mean 21.9±15.3 after HF hospitalization 0.4±0.9, no longer any beta-blockers, There cases lead dislodgement. In elevated PA pressure, good two-year achievable.Tabled 1Pre ablation*Post ablation**P-valueAF hospitalizations4.9 ± 5.30.4 0.9P<0.01On drugs7 (63.6%)0 (0%)P<0.01On rate control drugs11 (100%)0 (0%)NALead dislodgement-0 capture threshold, mV-0.7 0.2NAValues SD n (%) *24 **Average 22±15 Open table new tab

Benefits of Permanent His Bundle Pacing Combined With Atrioventricular Node Ablation in Atrial Fibrillation Patients With Heart Failure With Both Preserved and Reduced Left Ventricular Ejection Fraction
BACKGROUND Clinical benefits from His bundle pacing (HBP) in heart failure patients with preserved and reduced left ventricular ejection fraction are still inconclusive. This study evaluated clinical outcomes of permanent HBP in atrial fibrillation patients with narrow QRS who underwent atrioventricular node ablation for heart failure symptoms despite rate control by medication. METHODS AND R...
متن کاملPermanent His-bundle pacing after atrioventricular node ablation in a patient with chronic atrial fibrillation and mitral regurgitation.
Hemodynamic deterioration because of worsening of mitral regurgitation can occur in a small number of patients undergoing atrioventricular node (AVN) ablation and pacing therapy. Patients with moderate mitral regurgitation before ablation seem prone to this complication. Successful permanent His-bundle pacing after AVN ablation was performed in a patient with chronic atrial fibrillation and mod...

AIM To compare ventricular rate responsive (VVIR) pacing with dual chamber rate responsive (DDDR) pacing and antiarrhythmic drugs for the treatment of patients with persistent atrial fibrillation after atrioventricular node ablation. METHODS One hundred two patients with persistent atrial fibrillation eligible for the atrioventricular node ablation were randomly assigned to the therapy with e...
متن کاملProtective effects of simvastatin on atrioventricular node during simulated experimental atrial fibrillation in vitro
Introduction: Recent evidence has indicated that statins can reduce the incidence of both supraventricular and ventricular arrhythmias with various mechanisms. The primary goal of the present study was to determine direct protective role of simvastatin in modifying concealed conduction and the zone of concealment in a simulated model of atrial fibrillation (AF) in an isolated atrioventricula...
